If you are contacted by phone for an enrollment appointment, please be prepared to provide the following information:

• IEP (if applicable)
• Copy of Court Custody (if applicable)
• Immunization Record
• Proof of Address
• Proof of Income (1 month recent check stub)
• Medical Insurance Card
• Original birth certificate of all children living in the household
• Parent’s ID
